The commandant of the US Coast Guard has been removed from her position over “leadership deficiencies” and “operational failures,” a Department of Homeland Security official confirmed to CNN on Tuesday.

Coast Guard Commandant Adm. Linda Fagan has been fired from her role atop the U.S. military branch as President Trump's administration takes shape. Fagan was the first woman to have the leadership role.
